I will collect your personal data for the purpose of contacting you only for matters regarding your counselling sessions. It may become necessary to share your data if there is an immediate risk of substantial harm to yourself or others, or under a legal requirement for example Terrorism, Drug Money Laundering, via Court Order for disclosure or when there is a need to report a Safeguarding Issue.
Please note that except for the reasons mentioned above our sessions are confidential and we will discuss this in the usual way prior to our work together.
I keep written notes (on paper) to record the content of our sessions and these are stored anonymously in a locked filing cabinet. I am the only person who can access these. I store these records for 3 years and at the end of that time they are securely destroyed.
Your personal details are kept securely and separately from your anonymised records so you cannot be identified.
All contact details ( which you give me) will be stored for the duration of your counselling and for 3 years after that, but these forms will be stored separately from any records of the session (notes) so you cannot be identified.
For the purposes of matters associated with counselling appointments I may need to contact you by phone or e-mail but I will address you using first names only and will not store your full personal details in my phone, iPad or computer. The messages will be deleted as soon as they are no longer relevant (eg the appt is over).
I do NOT use your name, address, telephone number or email address for any kind of marketing.
The reason for recording information about the sessions is to help me to remember the session content, to remind me of the key things you have said and anything that I think is important to re-visit in future sessions. I keep notes for 3 years so that I have that information if you re-refer for counselling.
In the event of my death or serious illness that renders me unable to work on a permanent basis, my session records will be securely destroyed by a specific person already nominated by me.
Under GDPR you have rights as an individual which you can exercise in relation to the information I hold about you. If you wish to know about this please ask me.
